For official copies of your transcript, please follow the directions below.

Transcript Requests

To order an official Aviation High School transcript you must do the following:

  1. Print and complete these forms:   Transcript Request Forms.pdf 
  2. Make a copy of your passport, IDNYC, or state-issued photo ID with your signature.
  3. Pay for your transcript request at our school store  https://aviationhs.retailtime.cloud/or submit a check or money order along with your request by mail to 45-30  36th Street, Long Island City, NY 11101 
  4. Scan and email the completed forms to nrodriguez56@schools.nyc.gov  or fax your forms to 718-349-2787 or mail the completed forms to Aviation High School
    45-30 36th Street, Long Island City, NY 11101, Attn: Transcripts *Note: If you made an online payment include your invoice number

* Please only submit your request 1 time only using 1 of the delivery methods above. 

* If you are a third-party employment verification company you must send a release from the former student and the request MUST include the Full date of birth.
